Treating Children with Motor Speech Disorders

These treatment sessions can be conducted in as little as thirty to forty-five minutes. We have found them to be effective in children with motor speech disorders of varying types. A brief comment on changing from one stimulus to the other; we have found that it is advantages to use a "stairstepping" model for introducing new stimuli. That means that we never change the motor target, phonologic target, and the grammatic target in the same session. Only one new target is introduced in any given session. Once the child masters that target it is then introduced in a new context such as a more advanced grammatical structure or a more difficult sentence type. In this regard only, the information that you want the child to focus on is changed in any give session. In the preceding paragraph, I used the word context. I think that context is very important in developing effective therapy program for children with developmental speech disorders. My view is that apraxia is a disorder of motor organization/motor execution that is influenced by motoric and/or linguistic context. Clinicians who can master the ability to identify the context contributing to either success or non-success in speech production for these child can subsequently structure therapy using appropriate context to facilitate improved responses.

Children with childhood apraxia of speech (CAS) are frequently noted in the literature as having disordered prosody. Prosody refers to intonation, stress pattern, loudness variations, pausing, and rhythm. We express prosody mainly by varying pitch, loudness, and duration. We also may use greater articulatory force to emphasize a word or phrase. Children with CAS are frequently reported to have prosodic errors as well as many speech sound errors. Often, these prosodic errors and articulation errors are thought of as two separate entities. The purpose of this article is to highlight potential relationships between speech characteristics of CAS, as well as the efforts to improve speech sound accuracy, and their impact on prosody.

Lee; Masoyoshi Itoh Speech is a complex process involving visual and auditory input, central processing, and motor output, not only to the vocal muscles but also to the facial muscles. The facial muscles produce expression and are thus an integral part of vocal communication. The central processing of speech is also very complex, with input from primary and secondary visual and auditory areas, integration in the association areas (eg, Broca's area for word formation, Wernicke's area for language comprehension), and output through the primary and secondary motor areas. Moreover, the function of the same areas differs between the dominant and the nondominant hemispheres. Nonetheless, it is possible to associate lesions in certain areas with particular speech defects.
Aging and Speech
Speech patterns change with age: The voice becomes deeper and may develop an increasing tremor, speech becomes slower and may develop an abnormal prosody, and enunciation of consonants becomes imprecise. There is a slight increase in laryngeal muscle tension (glottal fold tension), which results in
