Extractions: Treatment for dermatomyositis generally consists of a steroid drug called prednisone. For patients in whom prednisone is not effective, other immunosuppressants such as azathioprine and methotrexate may be prescribed. Recently, a drug called intravenous immunoglobulin was shown to be effective and safe in the treatment of the disease. Extractions: Hello. My name is Sherrell and I am the mother of three loving children. My youngest child is a beautiful little girl named Jordan. She was only 10 years old, in 1998, when diagnosed with a rare disease called Juvenile Dermatomyositis. (JDMS) It is so rare, information is hard to come by. I thought I would put her diary on the Internet in hopes of getting more information and also, to let other parents, facing this same nightmare, know that they are not alone. The first months of the diary may be hard to read. It brings tears to my eyes and I wrote it and lived it. But, trust me, there are joyful tears in the months to come. We have been blessed, for God is taking care of our baby. We hope that the information is helpful to each of you. The diary is our personal experience and doesn't necessarily apply to everyone. However, if you share the diary with someone else that has been diagnosed with JDMS or is the parent of a child with JDMS, please encourage them to read the whole diary or at least through December 1998. I just recently found a forum that can connect us and may help answer many questions. You may want to check it out.

MDA's Search for a Cause, Treatment and Cure INTRODUCTION Polymyositis and dermatomyositis are related diseases, both involving inflammation swelling or irritation of the voluntary muscles, those that normally govern movement, such as the muscles of the arms and legs. Myositis means inflammation of muscle. Polymyositis means inflammation of many muscles, and dermatomyositis means inflammation of muscle and skin. The results of either condition are weakened muscles and often muscle pain. These conditions are seldom fatal, but in the most severe cases a person may need a wheelchair or require assistance with activities of daily living. As early as 1863, a disease involving muscle inflammation and a reddish rash on the face and upper body was described and given the name polymyositis. In 1887, an apparent case of polymyositis without any rash was described by the German physician P. Hepp. Another German, H. Unverricht, then proposed using the name dermatomyositis for cases of muscle inflammation with skin changes and polymyositis for those without them. This terminology remains in use today. Both conditions are among the 40 neuromuscular diseases in the Muscular Dystrophy Association's program.

Myositis literally means 'inflammation of the muscles'. This term covers a number of related illnesses including Dermatomyositis, Polymyositis, Juvenile Dermatomyositis and Inclusion Body Myositis. These diseases fall under the classification of 'Autoimmune' diseases, in that they are the result of abnormal immune responses (which the body would normally use to fight off diseases) attacking normal tissues - in this instance, the muscles. Thankfully these diseases are very rare, and to date, no-one really knows what causes them. As these diseases affect the muscles and connective tissues of the body, the main symptom is muscular weakness, which may be progressive and can be severely disabling, affecting in the majority of cases the whole body. People of any age and sex can develop 'Myositis', and depression and a general feeling of misery can frequently ensue. These diseases are not catching and there is no way to predict who may become affected. They can usually be suppressed by drugs which often have side effects which can promote worrying problems of their own. As yet there is no true cure for the disease, but many sufferers can go into partial or complete remission. Many sufferers need physiotherapy to prevent muscle shrinkage and to keep them mobile. Some fortunately do get better, but for the majority it is a case of coping with the disease and trying to live as near a normal life as possible. Some less fortunate do need a wheelchair from time to time while the disease is at its height.

Extractions: Here's some more info from Kim Schive ( KSchive@aol.com Dermatomyositis (DM), AKA sheltie skin syndrome, is a skin-and-muscle disorder that is known to affect collies (both varieties) and shelties, but is not generally seen in any other breed. Most of the research on canine DM (several varieties also occur in humans) has been done on collies, but it is likely that we are dealing with the same entity in both collies and shelties, so that research is relevant. The mode of inheritance appears to be autosomal dominant with variable expression. "Autosomal" means it is non-sex-linked, and occurs with equal frequency in males and females. "Dominant" means that one parent must HAVE IT to pass it on to offspring (or, possibly, it can occur as a new mutation; however, while possible, I believe this is highly unlikely, because if new mutations occured in canines, DM would be a problem in almost every breed). "Variable expression" means that disease severity can vary from very mildtoo mild to be diagnosedto very severe. Because the gene is dominant, there is no such thing as a "carrier." A dog either HAS it, or he doesn'twhether or not he gets diagnosed is the sticky part of the equation (see next paragraph). Affected dogs may have one gene for the disorder (D) and one normal gene (d). Such a dog would be referred to as heterozygous, and his genetic make-up would be Dd. Or, an affected dog may have two genes for the disorder (having inherited one from each parent). That dog would be referred to as homozygous, and his genetic make-up would be DD. It is probable that the more severely affected dogs are homozygous (DD), but nobody knows for sure.

Extractions: Bandolier Bandolier Library search Dermatomyositis occurs infrequently, with an incidence probably less than 1 per 300,000 per year. It is characterised by an intramuscular microangiopathy mediated by a complement membrane attack complex. There is a loss of muscle capillaries, muscle ischaemia, muscle-fibre necrosis and perifascicular atrophy. Treatment is usually with steroids, but cytotoxic drugs and immunosuppressants are also added. Many patients do not respond to this treatment and remain physically disabled. A very well conducted double-blind placebo-controlled randomised control trial of high dose immune globulin has shown that it is an effective treatment. The study from the NIH in Bethesda looked at fifteen patients with therapy resistant dermatomyositis. Patients were block randomised to immune globulin infusion or placebo by disease severity, and they continued on their normal therapy during the study. Immune globulin was infused at 2g/kg body weight, with one infusion each month.

First, to chronicle the events surrounding Julia's diagnosis and treatment of Juvenile Dermatomyositis, thereby helping me cope with it. Second, to give other children and their parents in the same situation some much-needed information and support.
