Extractions: Lymphedema is chronic swelling of a body part, most often an arm or leg, and often the unintended consequence of cancer surgery and/or radiation (secondary lymphedema). For many it is a congenital abnormality (primary lymphedema) presenting at birth, puberty or even mid-life. Lymphedema is often confused with lipedema, an abnormal accumulation of fat cells, which may, over time, develop into a secondary lymphedema or combination lipo/lymphedema. Lymph vessels are part of a one-way transport system which facilitates the return of tissue fluids back into the bloodstream. Along the way, this fluid is routed through lymph nodes which protect the body from infection and disease by storing and distributing the lymphocytes that attack dangerous pathogens. Metabolic waste and plasma proteins, which are too large to be returned, or reabsorbed by the venous capillaries, are also filtered via the lymph nodes. Anytime this lymphatic transport mechanism is compromised, whether by surgery, radiation, or congenital deficiency or malformation, stagnation and swelling will develop, proteins will accumulate, and the affected area will be at great risk for infection. The condition is progressive, and left untreated may develop into irreversible fibrosis (hardening of the tissue), chronic infection or loss of limb function, not to mention the deleterious effect on the patient's overall physical and emotional well-being.

Lymphedema causes swelling of the affected extremity; however, not all swollen extremities are caused by lymphedema. In congestive heart failure the heart does not work as well as it should. Think of the heart as a pump with an inlet and an outlet. When a pump does not work as well as it should, the amount of fluid pumped out by the pump is decreased. In addition to this, the amount of fluid that the pump is trying to remove remains high. Congestive heart failure is very similar. In congestive heart failure, the amount of blood pumped out by the heart is less than normal, decreasing the total amount of oxygen available to the tissues. Patients with congestive heart failure become easily fatigued because the heart cannot pump enough blood to satisfy the oxygen requirements during periods of exercise. Since the output from the heart is low, the input into the heart from the veins is also diminished and blood builds up behind the heart much like water builds up behind a dam.

That person is most likely suffering from lymphedema, a severely underaddressed issue relevant to women's health. It is an "orphan disease" with no specialists. And the Ace bandage is not what it appears. An Ace bandage is very elastic, while the lymphedema bandaging has very little stretch in it, but acts like a cast. When the muscle pushes, the bandage holds the skin in place, so that the tissue is squeezed and wrung out, like a sponge. Lymphedema is a chronic swelling, due to inadequate lymph drainage. You are at risk for developing lymphedema if you have had lymph nodes removed and/or radiation treatment following breast cancer. Lymphedema is not just an issue for women who have had breast cancer surgery, it is also seen in all kinds of pelvic tumors in both men and women. The number of men who suffer with lymphedema in the legs is high. It is also becoming apparent that plastic surgery patients, who have had liposuction, are at risk from the disturbance in the lymph tissue following the intrusion of the cannula. Poor circulation of lymph and localized fluid retention may be caused by a sedentary lifestyle or activities which involve prolonged periods of standing, causing swelling in the feet and ankles. In North America, the most serious forms of lymphedema most often result from lymph node removal or damage to the channels from radiation during cancer treatment. Primary lymphedema can be caused by a congenital deficiency of the lymphatics. Filariasis, a parasitic disease, is a major cause of lymphedema world-wide, but is rarely seen in temperate climates.

The human lymphatic system generally includes superficial or primary lymphatic vessels that form a complex dermal network of capillary-like channels that drain into larger, secondary lymphatic vessels located in the subdermal space. These primary and secondary lymphatic vessels parallel the superficial veins and drain into a third, deeper layer of lymphatic vessels located in the subcutaneous fat adjacent to the fascia. Active, unidirectional lymphatic flow in secondary and subcutaneous lymphatic vessels is aided by a muscular wall and numerous valves. Primary lymphatic vessels lack a muscular wall and do not have valves. An intramuscular system of lymphatic vessels that parallels the deep arteries and drains the muscular compartment, joints, and synovium also exists. While there is evidence that the superficial and deep lymphatic systems communicate near lymph nodes, they probably function independently except in abnormal states. [

The buildup of lymph causes swelling in specific areas of the body, usually an arm or leg, with an abnormally high amount of tissue proteins, chronic inflammation, and thickening and scarring of tissue under the skin. Lymphedema is a common complication of cancer and cancer treatment and can result in long-term physical, psychological, and social issues for patients. The lymphatic system collects excess fluid and proteins from the body tissues and carries them back to the bloodstream. Proteins and substances too big to move through the walls of veins can be picked up by the lymphatic vessels because they have thinner walls. Edema may occur when there is an increase in the amount of fluid, proteins, and other substances in the body tissues because of problems in the blood capillaries and veins or a blockage in the lymphatic system. Lymphedema may be either primary or secondary. Primary lymphedema is a rare inherited condition in which lymph nodes and lymph vessels are absent or abnormal. Secondary lymphedema can be caused by a blockage or cut in the lymphatic system, usually the lymph nodes in the groin area and the arm pit.
