Polymyositis means inflammation of many muscles, and dermatomyositis means inflammation of muscle and skin. The results of either condition are weakened muscles and often muscle pain. These conditions are seldom fatal, but in the most severe cases a person may need a wheelchair or require assistance with activities of daily living. Dermatomyositis Dermatomyositis (DM) is the most easily recognized of the inflammatory myopathies due to its distinctive rash. This rash occurs as a patchy, dusky, reddish or lilac rash on the eyelids, cheeks, and bridge of the nose, and on the back or upper chest, elbows, knees and knuckles. Some people with dermatomyositis develop calcified nodules or hardened bumps under the skin. The rash often precedes muscle weakness. Dermatomyositis can occur at any age from childhood to adulthood and is more common in females than males. High dose prednisone has been an effective treatment for many patients. Azathioprine and methotrexate are often used. Unfortunately, these drugs have adverse side effects, especially after prolonged use. Polymyositis Polymyositis (PM) does not have the characteristic rash of dermatomyositis (DM). Onset of muscle weakness usually progresses slower than DM. In the main muscles nearest to the trunk of the body are weakened whereas muscles furthest away are affected variable. Difficulty in swallowing is common in PM. Inability to breathe due to muscle failure is uncommon but occurs more often in PM than in dermatomyositis or inclusion body myositis. As many as one third of PM patients have muscle pain, but it is rarely the chief complaint. High doses of prednisone have been an effective treatment for many patients. azathioprine and methotrexate are also prescribed. Unfortunately, these drugs have adverse side affects, especially after prolonged use.

Extractions: Myositis Testing Laboratory Another type of autoimmune disease attacks the muscles of the body. The "myositis-specific antibodies" (MSA) can assist in the diagnosis of polymyositis and dermatomyositis in those patients who have the diseases. About 50% of patients with polymyositis or dermatomyositis have specific MSA or myositis associated antibodies (MAA). MSA are almost never found in patients without myositis, even if they have other muscle diseases of autoimmune diseases. This means that when the physician's examination and initial testing suggests the possibility of polymyositis or dermatomyositis, a positive test for an MSA can be strong supporting evidence for the diagnosis. For a long time the testing for these antibodies was only available in research studies, but it is now possible to obtain this testing clinically. The Myositis Profile, performed at the OMRF Clinical Immunology Laboratory, includes tests for 12 of the MSAs and MAAs. Additional antibodies may be detectable using this profile. Dr. Ira Targoff is the consultant to the Clinical Immunology Laboratory regarding this testing. As with the Reichlin Profile for lupus, patients with a particular antibody may have distinctive clinical pictures with certain associated problems. Identifying certain antibodies can help characterize the condition more fully. Research continues in our lab, and others around the world, to determine the nature of the relationship between the various MSAs and polymyositis and dermatomyositis.

When the disease spreads along the layers of tissue that surround muscle (the fascia), it is called necrotizing fasciitis . When the disease spreads into the muscle tissue, it is called necrotizing myositis Necrotizing fasciitis can be caused by a number of different bacteria, one of them being the group A streptococcus. This is a common bacteria which causes infections, such as sore or strep throat, in children and young adults. Ten to 15 per cent of school age children may carry group A streptococcus in their throat and have no symptoms. It is normally spread through close, personal contact with an infected person, for example, through kissing or sharing cutlery. This same bacteria also causes scarlet fever, impetigo and rheumatic fever. Sometimes group A streptococcus causes serious diseases such as pneumonia, streptococcal toxic-shock syndrome, and necrotizing fasciitis and myositis. In instances where serious disease develops, the presence of sore throat is very unusual.

What is inclusion body myositis? Inclusion body myositis (IBM) is a disease of muscle, which causes muscles to become thin and weak. It was recognised as a disease in its own right in the 1960's. It usually occurs in middle to late life and is more common in men than women. We don't know how many people in the UK are affected but it is the most common muscle disease diagnosed after the age of 50. Nevertheless, it is sufficiently rare that most general practitioners will not have looked after patients with IBM before, and many doctors will not have heard of the condition. How will it affect me? IBM is a slowly progressive condition causing a gradual deterioration in muscle strength over the years. Most limb muscles can be affected. In particular the quadriceps (the thigh muscles which extend the knee joint), and forearm muscles that flex the wrists and fingers are commonly weak. Accordingly, patients often notice difficulty with stairs, getting out of a chair and a poor grip. Swallowing muscles are affected in some patients, but most do not encounter severe swallowing problems. The disease typically does not affect muscles of the heart, eye, gut or bladder. It does not affect the function of the brain or sensation, and speech is rarely affected. In general patients do not die of the disease, but most meet with some degree of disability as the disease progresses.

Myositis ossificans progressiva, also known as Fibrodysplasia ossificans progressiva or FOP, is a disabling condition which is caused by the formation of bony bars within the muscles of the body. This bone formation is usually first noticed in early childhood as a series of hard lumps in the neck or along the spine. These lumps, which may be tender, gradually shrink in size as the affected muscles are replaced by bone. The appearance of bony lumps in muscles is usually spontaneous but can also be provoked by any injury to the muscles. Disability in MOP is physical and very variable in extent; intelligence in unaffected. How does the condition progress? MOP is progressive in that more muscles become involved with increasing age, but the rate of progression is very variable from one person to another. Furthermore, the condition tends to show long periods of inactivity (of up to several years in length). Can any muscles be affected?
