Inflammation may be caused by an infection. Acute prostatitis is characterized by the sudden onset of symptoms; chronic prostatitis causes persistent, long-term symptoms that are less severe. On gentle examination, the prostate is swollen and tender. The infection may spread from or into, the contiguous urogenital tract (epididymis, bladder, urethra), or the bloodstream. Perform a rectal examination and only once, gently palpate the prostate to see if it is tender, swollen, or edematous. Culture the urine to help identify the organism responsible (although there is no guarantee that the bacteria in the prostate will be in the urine). For patients 35 years and younger, treat for gonorrhea and other urethritis with ceftriaxone (Rocephin) 125mg im to1000mg iv and azithromycin (Zithromax) 1000mg po. For men over 35 years old, begin empirical treatment with ciprofloxacin 400mg iv, then 500mg po bid. Arrange for urological followup.

Prostatitis is inflammation of the prostate gland. Because women do not have a prostate gland, it is a condition only found in men. Protstatis may account for up to 25 percent of all office visits by young and middle-age men for complaints involving the genital and urinary systems. The term prostatitis actually encompasses four disorders: Acute bacterial prostatitis is the least common of the four types but also the easiest to diagnose and treat effectively. Men with this disease often have chills, fever, pain in the lower back and genital area, urinary frequency and urgency often at night, burning or painful urination, body aches, and a demonstrable infection of the urinary tract, as evidenced by white blood cells and bacteria in the urine. It is treated with an appropriate antibiotic. Chronic bacterial prostatitis is also relatively uncommon. It is acute prostatitis associated with an underlying defect in the prostate, a focal point for bacterial persistence in the urinary tract. Effective treatment usually requires identifying and removing the defect and then treating the infection with antibiotics. However, antibiotics often do not cure it.

We wished to catalog symptoms and etiologic factors for chronic prostatitis to gather epidemiological data about the disease. We also wished to determine the feasibility of doing this using the internet. METHODS: A 54 question survey was designed and posted to several internet newsgroups dealing with chronic prostatitis. The questions dealt with symptoms, predisposing factors, demographic data and sexual practices. Responses were collected by electronic mail and tabulated at a central site. RESULTS: We received 163 completed surveys over a 2 month period. Nine percent of responses came from individuals outside of the U.S. or Canada. Only 9.8% of completed surveys contained one or more unreadable answers. The responding population consisted of relatively young men (mean 43 years) with a chronic, relapsing set of symptoms mostly characterized by pain at various locations in the pelvis, irritative voiding symptoms and pain associated with ejaculation. Most respondents were potent and had a single sexual partner.

Over half the men could not name a single symptom they might experience if they had a prostate problem. Extractions: Prostatitis simply means inflammation of the prostate gland. It is the most common urological problem diagnosed in men under the age of fifty. It can be acute (acute prostatitis), or persistent or relapsing (chronic prostatitis). The term chronic pelvic pain syndrome is now often used instead of chronic prostatitis. In acute prostatitis the symptoms come on quite quickly and men feel very unwell. Pain is often felt in the lower back, the lower abdomen above the penis, and in between the legs. Fever, muscles aches and shivers, and problems passing water also usually occur. Although men of all ages may be affected it is slightly more common in younger men. Acute prostatitis is relatively uncommon and is usually associated with infection spreading from the bladder. It may also develop as a complication of sexually transmitted infections such as chlamydia and gonorrhoea. Chronic prostatitis is far more common than acute prostatitis. Although the problems with pain and passing water are the same as with acute prostatitis, in general these symptoms do not come on so suddenly although flare-ups do occur. Men with chronic prostatitis often experience very poor quality of life because of their relapsing or persistent symptoms. It can be caused by bacteria but this is uncommon and often no explanation for it can be found.

Debe evitarse el masaje prostático para reducir al mínimo el riesgo de bacteriemia. Puede haber exudado uretral si la prostatitis es secundaria a uretritis. Hallazgos de laboratorio . La confirmación del diagnóstico se establece por los resultados del análisis de orina, tinción de Gram y cultivo urinarios.
