You may not be aware of the female patient who regularly binges on thousands of calories and then forces herself to vomit, or of the teenaged boy who aims to eat less than 400 calories a day while exercising rigorously. It is estimated that in the United States, five to ten million women and up to one million men have an eating disorder. These may be underestimates, especially if one takes into account the diagnosis of binge eating disorder that was introduced in the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) of the American Psychiatric Association. Anorexia nervosa has the poorest prognosis of all the eating disorders; however, the prognosis is moderately improved if the disorder is detected and treated in its early stages. To that end, this article will highlight signs and symptoms that may be indicative of anorexia and interventions that are within the scope of the primary care physician. In addition, a brief description of some of the theories of anorexia's origins may help physicians to prevent the disorder in some patients. Theories about the causes of anorexia nervosa include both the psychological and biologic. One psychological theory is that food intake and weight are areas that a young woman can control in a life otherwise dictated by an overly involved family, which may include a parent who is unduely concerned with weight or appearance.
