In New York State, it is estimated that one out of every four elderly New Yorkers living at home is considered nutritionally at risk. This translates to a staggering 728,000 New Yorkers aged 60 or older at risk of malnutrition. Data on a national level equally demonstrate astonishingly high levels of hunger among the elderly. In a GAO report (August 2000) almost 2 million households aged 60 or older indicated that they either did not have enough of the right types of food needed to maintain their health or simply did not have enough to eat. Good health is closely linked to diet. When an elderly person reduces food intake due to health problems or financial restrictions, they in fact risk their ability to maintain independent, healthy lifestyles. Nutrition assistance programs are available under the Older Americans Act to reduce hunger for the elderly. More can be done to promote the nutrition assistance services available. The Food Stamp Program through the USDA is the primary, federal nutrition assistance program for low-income elderly New Yorkers that is always available for those who are eligible. Unfortunately, it is estimated only 17% of eligible seniors in upstate New York are enrolled in the Food Stamp Program.

Evidence That Prevention Works Compelling evidence from recent clinical trials supports the merits of aggressive risk reduction therapies for patients with CVD. The American Heart Association and the American College of Cardiology urge all health care settings where CVD patients are treated to develop specific protocols and procedures reminding health care providers to implement the guidelines and assess the success of appropriate treatments.

Providing such information is the goal of nutrition surveillance. The NYS Nutrition Surveillance Program is one of the first state-level surveillance efforts in the U.S., and is unique in its university-state agency partnership. It is nationally recognized for innovative efforts at both the state and local levels. The New York State Nutrition Surveillance Program at Cornell began in 1984 as a collaborative effort of the Division of Nutritional Sciences, Cornell University (DNS) and the Bureau of Nutrition, New York State Department of Health (DOH). In recent years, the Cornell group has worked to document nutrition issues and support effective programming, planning and policymaking in New York State. The DOH both funds and collaborates on the activities conducted by Cornell, and conducts other surveillance activities throughout New York State.

Extractions: Mayville, New York 14757 The Chautauqua County Office for the Aging, (CCOFA), a governmental Area Agency on Aging has, since 1973, maximized senior nutrition services with creative cooperative efforts, quality control and client-based programs. In 1996, the Senior Nutrition Program served 289,987 meals to 2,298 participants through direct congregate program operation and subcontracted home-delivered meals. Utilizing the 1997 budget of $1,276,822, we expect to continue our five-year policy of "no waiting lists/no one unserved" despite lessening funding, by actively continuing to work with meal suppliers, Jamestown Public Schools and Brooks Memorial Hospital and Meals on Wheels of Jamestown Area, Sinclairville 76ers and Dunkirk Fredonia Meals on Wheels. Participants in the 18 Congregate Nutrition Program "Dining Centers" may select a hot or cold meal (both supplying 1/3 of the RDA) with a 24 hour in advance reservation. Home-Delivered Meal participants receive a hot main meal and many also receive a cold meal delivery at the same time. Weekend shelf stable and frozen meals are available for the HDM participant as well as a four pack "Blizzard Box." With the establishment of the first congregate nutrition program in New York State in 1973, the Senior Nutrition Program set a precedent of client based services, that continues to the present. The first meal in New York State was served April 1, 1974. Demographically and philosophically different, the two primary population centers of Chautauqua County, Jamestown and Dunkirk-Fredonia have permitted, and at times, necessitated customized nutrition services. With only two population centers, the 1069 square miles of the county also require planning for very rural areas.
