      It is only during the last two decades that an appreciation has grown for the important role that nutrition plays in the prevention of chronic disease - including cardiovascular disease, diabetes, bone disease, and diseases which effect the eye, brain and kidneys. This recognition has substantially resulted from the work emerging from the HNRCA. There has also been an expanded appreciation of the effect of sound nutrition and exercise in health maintenance by preventing, for example, the muscle loss of aging. The mapping of the human genome has lead us to an exciting junction. In future decades we will take giant steps forward in understanding the role of nutrients in gene expression, and the role of genetic inheritance in defining nutrient requirements and the variability of dietary response. With this knowledge nutrition scientists will create diets to optimize the health of individuals. These dietary recommendations will ultimately have impact in reducing the medical care costs that are engendered in an aging population. Thus, we are poised, really, at a new frontier. The HNRCA is in the best position to continue its journey at the forefront of nutrition science research.

Since adequate nutrition is critical to health, functioning, and the quality of life, it is an important component of home and community-based services for older people. The ENP provides for congregate and home-delivered meals. These meals and other nutrition services are provided in a variety of settings, such as senior centers, schools, and in individual homes. The services help older participants to learn to shop for, and/or to plan and prepare, meals that are economical and which help to manage or ameliorate specific health problems as well as enhancing their health and well-being. The congregate meal programs also provide older people with positive social contacts with other seniors at the group meal sites. Volunteers who deliver meals to older persons who are homebound are encouraged to spend some time with the elderly. The volunteers also offer an important opportunity to check on the welfare of the homebound elderly and are encouraged to report any health or other problems that they may note during their visits. In addition to providing nutrition and nutrition-related services, the ENP provides an important link to other needed supportive in-home and community-based services such as homemaker-home health aide services, transportation, fitness programs, and even home repair and home modification programs.

    Aging produces physiologic changes that affect the need for several essential nutrients. While the impact of age-related alterations in physiology and metabolism has been extensively assessed in pharmacological studies, it has only been within the last two decades that much research has been conducted to define the impact of these changes on human nutritional requirements. The nutrition status of the elderly is also dependent on social conditions and is influenced by the long-term effects of chronic disease and the intake of medication, which can sometimes generate undesired interactions with nutrients.
    The physiologic changes of aging, including perceptual, endocrine, gastrointestinal, renal, and muscular changes, may affect nutrition needs.
  • Energy needs decrease with age as the result of changes in basal metabolism and physical activity. Basal need are not under the individual's control, but energy expenditure in physical exercise varies according to individual activity patterns. Physical activity can play a major role in maintaining energy balance.
  • Sensory impairments that occur with aging may result in consumption of a more monotonous diet. Taste and smeell dysfunction in older adults reveal a progressive decline with age.

    These initial programs were funded as part of a national pilot study to test the feasibility of developing a senior meals program. Congregate meal sites are located throughout the state in senior centers, schools, churches, community center, and other public or private facilities. Meals served at the sites are nutritionally balanced and must meet a minimum of 1/3 of the daily recommended dietary allowances (RDAs). In 1998, more than 21,000 older adults across the state benefited through the program, enjoying approximately 1.4 million meals. When older people can't leave their homes and can't personally prepare nutritious meals, home-delivered meals are an option. More than 8,000 older Nebraskans ate approximately 900,000 home-delivered meals in 1998. People who participate in either the congregate or home-delivered meal programs may make voluntary contributions toward the cost of the program. The state's Area Agencies on Aging are responsible for overseeing the congregate and home-delivered meals programs. Meals are served primarily to individuals who are aged 60 or older and their spouses. Additionally, meals are sometimes made available to people with disabilities or physical limitations who are under the age of 60.
