Diabetes and Diet 2/28/02 Four-and-a-half years ago, N'omi and I went on the Atkins diet (see note below). We are still on it, and still experiencing its benefits. From time to time, we take a few days off; it helps to "kick-start" the diet again.
At about four months into it, we had each lost almost 90 lbs. Over time, we have gained some of it back, but still weigh considerably less than we did when we started.
N'omi has not had any more heart attacks, which she had been having when we went on the diet. My mood swings have never returned.
There is a plateau effect; we're not quite sure how to overcome it. But it took years to get there.
9/2/97 As you might guess from my writings here, we are on everyone's mailing list when it comes to alternative health products. One day last month, we got one of those little books that goes at great length into a topic, and at the end offers you products or publications. This one was from Dr. Atkins, the man whose "Dr. Atkins' Diet Revolution" is not only one of the best-selling diet books of all time, it is one of the all-around all-time bestsellers, with 12 million copies out in 23 years, and still in print.
When N'omi and I read the booklet, we saw our symptoms. N'omi was having extensive leg problems, which we had identified principally as diabetic peripheral neuropathy. I was showing signs of hypoglycemia.

Diet, Exercise Can Stop Diabetes Before It Starts
New government estimates show 17 Million Americans already have diabetes, and 16 million more of us are likely to get it, unless we start taking preventive measures now. Diabetes. Most of us know something about it because, as Americans, we almost certainly know someone who has the disease – or we’re at risk for it ourselves. Diabetes is a metabolic disorder in which people have problems converting food to energy. It is the leading cause of adult blindness, kidney failure, and non-traumatic amputations, and a major cause of heart disease and stroke. Last month the US Department of Health and Human Services (HHS) released an updated estimate – based on population changes in the most recent U.S. census – indicating that the number of Americans with diabetes has ballooned to 17 million. This represents an increase of 8% from the most commonly-used previous estimate, and another step in the wrong direction for a population increasingly plagued by diseases that reflect our less-than-healthy lifestyles. In addition, HHS Secretary Tommy G. Thompson warned Americans of the risks of "pre-diabetes," a condition affecting nearly 16 million Americans that sharply raises the risk for developing Type-2 diabetes and increases the risk of heart disease by 50%. (Type-2 diabetes used to be called “Adult Diabetes” because of its preponderance in the older population, but even that parameter is becoming blurred by the increasing number of young people who acquire Type-2 diabetes.)

In November of 1998, Joslin Diabetes Center began a campaign to educate people with diabetes and the general public about what the nutrition issues are for people with diabetes. This campaign includes a web-based discussion group hosted by Chalmers at www.joslin.org, that will answer questions about diabetes and diet. Anyone can pose a comment or question to the discussion group. New guidelines for people with diabetes were issued in the Spring of 1994 by an American Diabetes Association committee on nutrition, co-chaired by Joslin Vice President Edward S. Horton, M.D. Those guidelines state that it is okay for people with diabetes to substitute sugar-containing food for other carbohydrates as part of a balanced meal plan.

Why should I be concerned about diabetes? In the United States, Type 2 diabetes (also called adult-onset diabetes) is reaching epidemic proportions. It has even reached an alarming number of teenagers and young adults, a group that seemed practically immune to the disease just 20 years ago. There's no mystery behind this increase in incidence. Scientists don't need to explore various theories or perform experiments to understand the problem. The reason for our national struggle with diabetes is as obvious as our lifestyle. In general, our diets, activity levels, and waistlines have all taken an unhealthy turn, and Type 2 diabetes is the price many of us end up paying. The good news is that neither your lifestyle nor your risk of developing diabetes is written in stone. You can buck the national trends by exercising regularly, eating a well-balanced diet, and watching your weight. In fact, people at risk of Type 2 diabetes can more than halve their risk of developing the disease by exercising about half an hour a day and adopting a low-fat diet, according to a study conducted by the National Institutes of Health that was released in 2001. Participants who did about 30 minutes of walking or other low-intensity exercise a day, coupled with a low-fat diet, lost an an average of 5 to 7 percent of their body weight and cut their chances of developing Type 2 diabetes by 58 percent. Those treated with the diabetes drug metformin but who didn't make the lifestyle changes cut their risk by only 31 percent.

Endocrinologists have focused much of their research on diabetes and its causes.

While they continue to seek better ways of controlling diabetes, researchers have discovered that a condition known as glucose intolerance appears to serve as an early warning sign of type 2 diabetes. Experts estimate that more than 20 million people in the United States have glucose intolerance.
Recent studies have revealed that glucose intolerance is most prevalent in people who are overweight, lead a sedentary lifestyle and have a diet high in fat and carbohydrates. Genetics also appears to play a role as many people who eventually discover they have diabetes reveal that close family members also have it.
An alarming rise in the number of cases of type 2 diabetes in children has prompted endocrinologists and other researchers to study factors that put children at risk for the condition. Measuring glucose levels in the blood of children who are overweight reveals a high incidence of glucose intolerance. Clearly, the importance of new treatments for glucose intolerance cannot be overstated.
Clinical Trials: Their Value and Effect
Because the link between glucose intolerance and the eventual development of diabetes is relatively new, physicians have few pharmaceutical treatments for the condition. Generally, they suggest diet and exercise. In fact, a loss of even ten pounds tends to help control the condition. But few people have success with diet and exercise programs over the long term.

Andrew Nicholson, M.D. Diabetes is not necessarily a one-way street. Early studies suggest that persons with diabetes can improve and, in some cases, even cure themselves of the disease by switching to an unrefined, vegan diet. Unfortunately, none of these studies included a comparison group. So the Diabetes Action and Research Education Foundation provided a grant to PCRM to perform a carefully controlled test. Working with Georgetown University, we compared two different diets: a high-fiber, low-fat, vegan diet and the more commonly used American Diabetes Association (ADA) diet. We invited persons with non-insulin-dependent diabetes and their spouses or partners to follow one of the two diets for three months. Caterers prepared take-home lunches and dinners, so participants could simply heat up the food at home. The vegan meals were made from unrefined vegetables, grains, beans, and fruits, with no refined ingredients, such as vegetable oil, white flour, or white pasta. These meals averaged just 10 percent fat (as a percentage of calories) and 80 percent complex carbohydrate. They also offered 60-70 grams of fiber per day and had no cholesterol at all. The comparison (ADA) diet contained somewhat more plant-based ingredients than the average American diet, but still relied on the conventional chicken and fish recipes. This diet was 30 percent fat and 50 percent carbohydrate. It provided about 30 grams of fiber and 200 milligrams of cholesterol per day.

Upcoming Diabetes Study January 20, 2003, issue of Newsweek . The study, headed by Neal D. Barnard, M.D., and to be conducted along with investigators from the George Washington University and the University of Toronto, randomly assigns 68 individuals with type 2 diabetes to either a low-fat, vegan diet or a diet based on American Diabetes Association guidelines, while holding exercise constant. Recruitment for the study, which will be conducted in Washington, D.C., will not begin until September 2003. Individuals with diabetes should not wait until then to make diet changes in consultation with their personal physicians. Diet and exercise are essential in diabetes management.
