Nutrition and Alternative Therapies in Liver Disease

Nutrition
Patients suffering from any liver condition may benefit from adequate nutrition and dietary supplements. Unfortunately, many patients with liver disease believe that they are restricted and condemned to eat little of what they like for the rest of their lives. At the same time, some diets promise too much benefit and have not been proven to change the natural course of the liver disease. A detailed summary of recommendations is beyond the scope of this brief review. Patients should always discuss nutritional issues with their physicians and look for expert advice. Beware of the commercial interests behind the advertisements of the local or the national chain "nutrition store." Carbohydrates are rarely a problem and liver patients have no particular restrictions, except when other coexisting conditions are present (obesity, diabetes). Proteins
Restriction of protein has been traditionally recommended for patients with cirrhosis based on the fear that excess proteins would precipitate a condition called encephalopathy (changes in mental status). In reality, excess protein intake with diet is not a common precipitant of encephalopathy, as more often there are other reasons for this complication. Therefore, for most patients there are no protein restrictions. When patients have repetitive episodes of encephalopathy, a 40 grams per day protein restriction is issued for the first 48 hours. In general, this can be followed by unrestricted vegetable protein. The total daily dose of any type of protein can be increased to about 0.8-1 gram per kilogram (2.2 lbs.) of body weight daily. Medical treatment of encephalopathy is required in addition to the diet.

Dysphagia is the most common cause of death in HD, either directly from suffocation or aspiration, or indirectly from starvation.
(From A Physician's Guide to the Management of Huntington's Disease by Neal G. Ranen, M.D., Carol E. Peyser, M.D., and Susan E. Folstein, M.D., published 1993 by the Huntington's Disease Society of America, p. 24.) Dysphagia means difficulty in swallowing. According to A Physician's Guide to the Management of Huntington's Disease dysphagia in Huntington's Disease appears to result from "difficulty with closing the mouth and chewing, mucus formation, choking on liquids, food gulping, difficulty with coordinating the sequence of swallowing, difficulty clearing the mouth of excess food after each swallow, and the unexpected inspiration of air." A new pamphlet entitled Huntington's Disease , published in 1996 by the Huntington's Disease Society of America, says on p. 17, "Nutrition is important in everyone's life, but takes on added significance in HD. People with HD require an unusually high number of calories to maintain their body weight. Maintaining, or even gaining, weight can help reduce involuntary movements and other symptoms, particularly in the later stages of HD." For this reason, we are trying to gather together cookbooks, recipes, ideas for finger foods, etc. for people with Huntington's Disease. edicine fascinates me because it constantly teaches me something new. Back in the 1850's, the German philosopher Goethe said, "what you know about you see." Once you know about something you start seeing it everywhere, and this is certainly true about medicine and nutrition. I want to increase your understanding of diet and nutrition so you start seeing things differently, especially when it comes time to order your next meal in a restaurant or when you are pushing your cart down the supermarket aisles trying to decide what to bring home for dinner. Nutritional Awakening It began when I was on the cardiovascular anesthesia service (a service that deals with people's hearts and blood vessels). One evening I was making a round to see a patient who was scheduled for a four-vessel coronary by-pass in the next few days. The man had been out on a day-pass with his family before surgery, quite a common practice and had returned to the hospital rather late. The nurse notified me that there were several blood tests - missing from his chart, so we promptly performed another blood draw and -brought it to the nurse's station. I let it sit there while I got busy with some work in the ward. An hour and a half later I went back to the nurse's station to take the blood tube down to the laboratory. What I saw in the blood tube gave me a rude awakening as to the condition of this patient's coronary health.
