Phlebitis is almost always accompanied by a blood clot, or thrombus, in the affected vein, a condition known as thrombophlebitis (see thrombosis ). Blood clot formation may follow injury or be associated with infections. Thrombophlebitis of deep veins, usually in the legs or pelvis, may occur in patients recovering from childbirth, surgery, or other conditions requiring prolonged bedrest; the clotting mechanism is thought to be impaired when the legs are immobilized. Pregnancy or the use of oral contraceptives predisposes some women to thrombophlebitis. The major danger is that a clot originating in the leg vein may dislodge and travel to the lung, a condition known as pulmonary embolism (see embolus ). To avoid the risk of embolism, thrombophlebitis is usually treated with

Answer: The correct answer is letter 'a'. Fortunately, the popliteal vein of this patient was clear, but the veins peripheral to it were thrombosed. The patient was admitted and placed on low molecular weight heparin. This patient had multiple risk factors for deep vein thrombosis, including obesity, immobility and the use of oral contraceptives. With regards to the other answers, the patient's exam was consistent with a cellulitis, but the history was wrong for this condition. We don't obtain leading edge cultures in simple cellulitis due to the low yield. An ASO titre and throat culture would be appropriate if rheumatic fever was suspected, but the rash shown is not consistent with Erythema Marginatum, which has serpiginous borders. An RPR would be obtained if syphilis was suspected, but the rash of secondary syphilis is not tender or warm. © Jay D. Fisher MD

89. Superficial Phlebitis
What to Do for SUPERFICIAL phlebitis. That's it. You don't need antibiotics, because there is rarely any infectious component to phlebitis.
http://www.legsavers.com/phlebitis.htm
What to Do for SUPERFICIAL PHLEBITIS REST FOR A COUPLE OF DAYS until the symptoms subside. KEEP THE LEGS ELEVATED and apply warm compresses. DON'T RUB THE AFFECTED AREA. It can dislodge a clot in a deeper vein. WHILE YOU ARE IN BED, move your feet, ankles and legs from time to time to stimulate the circulation. ELASTIC STOCKINGS can relieve your discomfort and help the flow of blood by increasing muscle tone. FOR PAIN RELIEF take aspirin or ibuprofen as needed. That's it. You don't need antibiotics, because there is rarely any infectious component to phlebitis. ONCE THE ATTACK IS OVER RESUME YOUR NORMAL ACTIVITIES gradually, but... GET PLENTY OF REST. POSTPONE TRAVEL PLANS for a few weeks if they involve long flights or car trips. IF YOU'RE ON THE PILL, consider some other form of birth control. STOP SMOKING. TRY NOT TO CROSS YOUR LEGS when sitting.

Definitions Phlebology – the branch of medicine and surgery dedicated exclusively to the diagnosis and treatment of vein related problems. Phlebitis – also known as thrombophlebitis, phlebothrombosis, or superficial phlebitis – an inflammation of a vein with subsequent development of a clot (thrombus) within its lumen. Often a very painful condition. Phlebectomy – the surgical removal of a vein or vein segment, accomplished by a variety of techniques. Spider veins – also known as telangiectasia – small venules, arterioles or capillaries that appear on the leg. Their color varies depending on the oxygen level of the blood filling them. Varicose veins – veins that have become dilated and tortuous. They are generally at least several millimeters in diameter. Sclerotherapy – the use of a sclerosant solution injected into a small vein to cause it to disappear. The solution acts on the lining cells of the vessel to produce a thrombus; once the body absorbs this, the vessel disappears since blood no longer flows through it. (See Introduction) Varicose ulcer – the breakdown of skin, generally in the lower third of the calf above the ankle level, due to stasis (pooling) of blood flow in the area. The blood is poorly oxygenated and eventually leads to skin breakdown (ulcer).

Thrombosis is the formation of a blood clot that may partially or completely block a blood vessel. A clot located in an inflamed, superficial blood vessel is called thrombophlebitis. A clot formation in a deeper vein is called a deep vein thrombosis. Blood clots may follow injury, surgery, childbirth, or prolonged bed rest. They may also be associated with infection. Symptoms of phlebitis may include:
* a hard and cord-like vein that is very sensitive to pressure * a surrounding area that is red and warm to the touch or * a slight fever The primary danger in thrombophlebitis is that the clot may move and travel to a vital organ, such as the lung, causing serious damage.

I.V. phlebitis Intravenous line phlebitis is often referred to by nursing staff as tissuing of the line, although the latter term is often extended to imply any cause of dysfunction of lines. The phlebitis can be a cause of postoperative fever. Incidence can be lowered by changing sites every one or two days, although this may be difficult practically, and it may be better to change site when the patient begins to notice discomfort or the adjacent skin begins to redden.

Dakota at 3 yrs. old.
These pictures were taken November 2001. Dakota and Phlebitis On February 18, 2002, Dakota had her teeth cleaned. Not a big deal except for the fact she was put under anesthesia for the procedure. The anesthesia was administered via a catheter inserted into her right foreleg. After the catheter was removed, Dakota developed a swelling in the area and presented with a slight limp. I was told by veterinarian personnel that some swelling was normal. The limp was possibly caused by the fact Dakota is such a pain in the neck to handle and her leg may have been held too hard and bruised. Her limp became progressively worse during the week. I examined her leg and noticed the leg felt extremely hot to the touch. I put her in the tub and allowed tepid water to run over her leg to bring down the fever. The next morning, the leg was raw looking and little patches of hair were missing. Dakota had been biting at it while in her crate the night before. I didn't like the way it looked, or felt, so I took her into the vet.

Phlebitis is an inflammation of a vein. Thrombosis is the formation of a blood clot that may partially or completely block a blood vessel. A clot located in an inflamed, blood vessel is called thrombophlebitis. A clot formation in a deeper vein is called a deep vein thrombosis. Blood clots may follow injury, surgery, childbirth, or prolonged bed rest. They may also be associated with infection. SYMPTOMS of phlebitis may include: * a hard and cord-like vein that is very sensitive to pressure or touch * a surrounding area that is red and warm to the touch * a slight fever or * swelling of the extremity The PRIMARY DANGER in thrombophlebitis is that the clot may move and travel to a vital organ, such as the lung, causing serious damage. If you have symptoms of thrombophlebitis or deep vein thrombosis your healthcare provider may order a doppler ultrasound examination (a test that uses sound waves to locate clots); or a venogram in which dye is injected into a vein, usually in the foot. X-rays are then taken to visualize the circulation and any blockages that may be present. Lung scans may be ordered if there is concern that blood clots have traveled to the lungs. Thrombosis is TREATED with anticoagulant drugs, such as heparin, coumadin, dalteparin, enoxaparin, and tinzaprin. These drugs inhibit the formation of blood clots. Bed rest with local heat and elevation of the affected extremity are usually required.
