All disorders listed in the DSM in some way disturb the process of arousal and the sexual response cycle . Although controversial, it is the standard approach used by many psychiatrists and clinicians in the United States and other countries to female sexual problems. Hypoactive sexual desire disorder is characterized by an absence of libido. There is no interest in initiating sex and little desire to seek stimulation.

Hypoactive Sexual Desire
Integrating Sex and Couple Therapy Gerald R. Weeks and Nancy Gambescia Overview Table of Contents The lack of sexual desire, known clinically as hypoactive sexual desire (HSD), is generally recognized as the most common sexual problem in America. Over 50% of couples presenting for treatment will complain of insufficient sexual desire in one or both partners. Thus, all clinicians are very likely to encounter HSD in both individual and couple therapy practices. Here, Gerald Weeks and Nancy Gambescia present a treatment model for HSD based on the integration of medical and psychological interventions. This book provides clinicians with the theoretical and practical tools to understand and treat this complex problem. The book opens by providing a general background about HSD and describing the overall framework of the problem. The authors then review theories about the presence, absence, and normative amounts of sexual desire and cover factors that contribute to the lack of [sexual] desire from the individual perspective. Both nonpsychiatric and psychiatric factors are examined.

Sexual Pharmacology: Fast Facts Robert Taylor Segraves and Richard Balon Overview Table of Contents Sexual side effects of pharmacological agents are often an unspoken cause of treatment noncompliance. Controlled studies in the United States, UK, and Spain have consistently found that approximately 75% of patients who experience sexual side effects from prescribed medication will not volunteer this information without direct questioning. Nonetheless, many clinicians do not have easy access to reliable information concerning sexual side effects of pharmacological agents. This handy reference provides up-to-date and rapid access to the latest available data concerning sexual side effects of various pharmacological agents for psychiatrists and physicians. They are dysfunctions when they case personal distress and/or interpersonal (social) difficulties. They can arise in the area of desire, excitement, orgasm, and/or resolution. They can be of lifelong or acquired duration and can be generalized to many situations or situational and occur only in specific situations. They can be due to psychological factors or due to a combined physical and psychological problem.

N E W S F E A T U R E

On Nov. 30, at 2:00 a.m., John Hardy Rose is scheduled to be executed at Central Prison in Raleigh. Rose was convicted of the Jan. 3, 1991 murder of Patricia Stewart. Rose broke into Stewart's apartment, stabbed her five times and stuffed her body into the trunk of a car. The following evening, he drove to his grandmother's property, burned Stewart's body and buried her in a shallow grave.

During an interview on death row, five days before his scheduled execution, John Hardy Rose said he's at peace with his decision not to seek clemency.
Shock reverberated through rural Graham County with news of the murder. The county of some 8,000 sits in the northwestern corner of the state. Robbinsville, the county seat, rests against the Joyce Kilmer Forest amid the Great Smoky Mountains through which Rose wandered as a boy. His father was an abusive man, an alcoholic who beat his wife and on occasion would force his boy to have sex, while he watched, with adult women. The family was so deeply impoverished that Rose's mother would sometimes have to beg for food. They moved often, allegedly 21 times in a single year.

Volume 33, No. 6 June 2002 Can psychology help a church in crisis? A small group of psychologists says yes, asserting that psychology's science and practice can be part of the solution. BY JENNIFER DAW

The news reports of Catholic priests charged with sexually abusing adolescents have shocked thousands of Americans. But for a group of psychological experts, the recent events aren't just news, they're a call for help from psychology. "Psychology can offer so much to the church, the public, victims and rank-and-file Catholics, as well as priests," says Thomas Plante, PhD, editor of "Bless Me Father for I Have Sinned: Perspectives on Sexual Abuse Committed by Roman Catholic Priests" (Greenwood, 1999) and professor and chair of psychology at Santa Clara University. "We can offer the best in science and the best in practice and let data cut through the hysteria," says Plante, who evaluates those entering religious life for necessary characteristics and for potential red flags, and treats victims and offenders in his practice. Though the number of psychologists conducting research and treating clergy is small"there's only about 15 of us in the Northeast," says Plantepsychologists can help prevent further abuse by recognizing risk factors in potential priests and collaborating with religious superiors.

Although this is, strictly speaking, not a sexual dysfunction, it is a disorder that can severely disrupt the sexual relationship of a couple. ISD is the persistent and pervasive inhibition of sexual desire. ISD is present if there is both a low rate of sexual activity and a subjective lack of desire for sexual activity; desire here includes sexual dreams and fantasies, attention to erotic material, awareness of wishes for sexual activity, noticing attractive potential partners, and feelings of frustration if deprived of sex. The DSM IV describes Hypoactive Sexual Desire Disorder as:
  • Persistent or recurrent deficient (or absent) sexual fantasies and desire for sexual activity. The judgment of deficiency or absence is made by the clinician, taking into account factors that affect sexual functioning, such as age, sex, and the context of the person's life."

Disorders
Sexual disorders include problems of sexual identity, sexual performance, and sexual aim. There are three major catergories of sexual disorders: sexual dysfunctions paraphilia , and gender identity disorders
Sexual Dysfunctions Sexual dysfunctions prevent or reduce an individual's enjoyment of normal sex and prevent or reduce the normal physiological changes brought on normally by sexual arousal. These dysfunctions can be classified by the phase of the sexual cycle in which they occur. It is important to keep in mind that the diagnosis of sexual dysfunction is made only when the disability persists. Any of them could occur occasionally or be caused by a temporary factor such as fatigue, sickness, alcohol, or drugs.
The Desire Phase There are two types of dysfunctions that can occur during the desire phase. One is hypoactive desire which is basically a disinterest in sexual activity. It results in a complete or almost complete lack of desire to have any type of sexual relation. This can often result in the participation in intercourse as a simple marital duty. The second type is an aversion to sex This is different from simple hypoactive sexual desire in that sexual activity actually repulses the person or makes them unusually apprehensive. This is most often the result of a traumatic sexual experience, such as molestation as a child or rape.

http://www.addictionresearch.com/vanderbilt.cfm
In partnership with The Homewood Foundation, the American Foundation for Addition Research and Vanderbilt University have begun the initial phase of a unique five-year research project to learn more about the relationship of sexual arousal, brain chemistry, and addiction, and the effectiveness of selective serotonin reuptake inhibiting (SSRI) medications such as Prozac, Zoloft, and Paxel. The initial phase of this unique joint research project began in January 2000. "The objective of the study’s first phase is to develop a Functional Magnetic Resonance Imaging (fMRI) map of the brain regions that are activated during sexual arousal in normal male volunteers as a precursor in studies in patients with sexual arousal disorders," said Vanderbilt’s Peter Martin, M.D., the project’s director. fMRI imaging provides non-invasive high resolution activation maps of the visual cortex, motor cortex, and other brain systems, including those activated with administration of drugs of abuse. In the study’s second phase, researchers hope to (1) determine the fMRI map of the brain regions that are activated during sexual arousal in normal female volunteers as a precursor I patients with sexual arousal disorders, and (2), determine the fMRI map of the brain regions that are activated during sexual arousal and cue-induced cocaine or alcohol craving in male and female patients who meet research diagnostic criteria for sexual addiction and cocaine or alcohol dependence. "This project also seeks to identify the neurobiological basis of sexual addiction and its overlap with other addictive disorders, such as alcohol and cocaine dependence, and to identify pharmacological agents that may improve treatment outcome," said Elizabeth Griffin, AFAR Associate Director. Thus, in the study’s final phase, researchers will work to determine the effectiveness of SSRIs in patients with sexually compulsive behaviors.

What Is Sexual Addiction?
Michael Herkov, Ph.D., Mark S. Gold, M.D., and Drew W. Edwards, M.S.
6 Feb 2001
Sexual addiction is best described as a progressive intimacy disorder characterized by compulsive sexual thoughts and acts. Like all addictions, its negative impact on the addict and on family members increases as the disorder progresses. Over time, the addict usually has to intensify the addictive behavior to achieve the same results. For some sex addicts, behavior does not progress beyond compulsive masturbation or the extensive use of pornography or phone or computer sex services. For others, addiction can involve illegal activities such as exhibitionism, voyeurism, obscene phone calls, child molestation or rape. Sex addicts do not necessarily become sex offenders. Moreover, not all sex offenders are sex addicts. Roughly 55 percent of convicted sex offenders can be considered sex addicts. About 71 percent of child molesters are sex addicts. For many, their problems are so severe that imprisonment is the only way to ensure society's safety against them. Society has accepted that sex offenders act not for sexual gratification, but rather out of a disturbed need for power, dominance, control or revenge, or a perverted expression of anger. More recently, however, an awareness of brain changes and brain reward associated with sexual behavior has led us to understand that there are also powerful sexual drives that motivate sex offenses.
