SEXUAL DISORDERS
The Sexual Disorders are divided into two groups. The Paraphilias are characterized by arousal in response to sexual objects or situations that are not part of normative arousalactivity patterns and that in varying degrees may interfere with the capacity for reciprocal, affectionate sexual activity. The Sexual Dysfunctions are characterized by inhibitions in sexual desire or the psychophysiologic changes that characterize the sexual response cycle. Finally, there is a residual class, Other Sexual Disorders, for disorders in sexual functioning that are not classifiable in any of the specific categories.
PARAPHILIAS
The essential feature of disorders in this subclass is recurrent intense sexual urges and sexually arousing fantasies generally involving either (1) nonhuman objects, (2) the suffering or humiliation of oneself or one's partner (not merely simulated), or (3) children or other nonconsenting persons. The diagnosis is made only if the person has acted on these urges, or is markedly distressed by them. In other classifications these disorders are referred to as Sexual Deviations. The term Paraphilia is preferable because it correctly emphasizes that the deviation (para) lies in that to which the person is attracted (philia).

Mentioned in the National Service Framework for Mental Health as a support to strengthen primary mental health care, the WHO Guide to Mental Health in Primary Care has been prepared in the light of the experience of primary care clinicians and takes account of their specific needs. Short, user-friendly and oriented towards management it contains essential information for both General Practitioners and other staff on how to help adults suffering mental ill health, providing guidance on:
diagnosis and treatment
what to say to patients and their families
how to give them support and advice
It incorporates materials to support clinicians working collaboratively with patients, encouraging patients' active participation in treatment. A set of patient information and self-help leaflets is provided on disk. Summary cards are also included which are designed for interactive use between the clinician and patient in order to facilitate discussion. Each copy of the book also includes a second disk, which contains the text of the guidance. This enables primary care organisations to produce and use their own localised version of the Guide.
The evidence base for the recommendations on treatments is set out. However, the guidance is based on much more than the outcomes of trials of efficacy of particular treatments. It represents also the clinical wisdom of a very large group of experts whose collective experience of mental health in primary care is enormous.

The aims of this site. Child hood sexual abuse is far more common than many would like to think. Here in the UK it works out at just over 400 people a day abused for the first time. It usually happens with someone known to the child, and trusted by the family, and in very many case's is even a member of the child's own family. It happens to both females and males, and the abuser can also be either male or female and often is done by both. Sometimes it happens just the once, but sadly it often goes on for weeks or even many years.

Department of Pediatric Urology, "Aghia Sophia" Children's Hospital, Athens, Greece Key words: Ambivalent genitalia, Hermaphroditism, Pseudohermaphroditism, Sexual differentiation * The views expressed in this article have been proposed by the author to the Health Care Office of the European Association of Urology as guidelines for the management of intersex patients. o ne of the most challenging problems a urologist has to face is the diagnosis and management of patients with sexual differentiation disorders. Such disorders most often manifest themselves by the abnormal appearance of the external genitalia when the baby is born, casting doubt on the baby’s gender and causing a lot of anxiety on the part of the family. Problems of ambiguous genitalia should, in principle, be managed by a team of specialists in order to reach a prompt and correct diagnosis. Urologists have an important role is this group and should work in close cooperation together with neonatologists, pediatric endocrinologists and geneticists, in order to achieve the best possible management of these patients. CHOICES was established in 1981. Since that time we have provided treatment to a broad range of youth 8 to 18, and adults with sexual behavior disturbances. Specialized intervention services have also been developed for lower income individuals in cooperation with the Eugene Family Institute through the establishment of a specialized training program for therapists choosing to learn and work with these specialized client needs. CHOICES provides comprehensive psychological evaluations of individuals who are either accused of or convicted of sexually abusive behaviors as part of our effort to develop an understanding of the specific needs of each individual. Individual, group, marital and family therapy are available and employed as indicated by the client's needs. We also have a complete sexual arousal assessment facility to aid in the assessment and treatment of extremely obsessive or compulsive sexual disorders.
