vCJD is the human form of mad cow disease
By the BBC's Matt McGrath The victims of vCJD may have succumbed to the disease because they ate contaminated beef when they were suffering from a bout of tonsillitis or a simple sore throat.
Could it be that these patients had a sore throat and they were just unlucky that they ate a sausage or a meat pie that had a lot of BSE brain associated with it? Professor Stephen DeArmond, UCSF The theory has been put forward by a leading American scientist, Professor Stephen DeArmond, from the University of California at San Francisco. He said it could explain why relatively few people had been struck down with the illness so far and why so many of those who had were under the age of 35. British researchers studying vCJD said the theory was plausible but stressed there was no experimental data to support it.

  • Sore throat is a diagnostic challenge. Unlike other algorithms in this series, sore throat 1 provides some diagnostic information. Sore throat 2 is a treatment algorithm. Overview of Acute Sore Throat An acute sore throat is a very common problem. Most upper respiratory tract viral infections begin with a sore throat. Unfortunately many Americans bring virtually every sore throat to their physician. The key is to correctly diagnose bacterial tonsillitis, treat this with antibiotics, correctly diagnose viral infections, not treat these with antibiotics and then recognize (preferably early) sore throat illnesses such as: tonsillitis, gonorrhea, peritonsillar abscess, mononucleosis, candidiasis, tumor etc.

    Consultation for Sore Throat
This consultation is part of a series of consultations with Head and Neck Surgery specialists at the University of California, San Diego. The case is a hypothetical patient chosen to represent a composite of the usual and most common patients with this specific disorder. Where gender, age or race make a difference, these will be specifically cited. Where they do not make a difference, they may be omitted. The consultation is presented for purposes of general information. Specifics about an individual case and specific treatment must be discussed between the patient and the treating physician. The patient is an 18 year old junior college student who self-refers for recurrent sore throats.
Doctor : Good Morning. What can I do for you today? Patient : I am having recurrent sore throats and they are interfering with my ability to study and to work. I would like to have my tonsils out. Doctor : Tell me about your sore throats. Patient : Well, they come on once a month. I usually go to the doctor. They always give you some pills. The sore throats typically go away, but I always miss a couple days work, I always feel sick, my throat hurts and I am tired of living this way.

Pharyngitis, called sore throat, is the inflammation of the pharynx (throat). The throat extends from the nasal passages above and behind the mouth to the esophagus (tube that carries food to the stomach) in the neck. Pharyngitis occurs most commonly with a viral upper respiratory infection (URI). It is also symptomatic of a number of diseases, including:
    • diphtheria,
    • mononucleosis caused by Epstein-Barr virus,
    • gonorrhea , and
    • acute HIV infection.
Incidence and Prevalence Approximately 40 – 60% of cases of pharyngitis are caused by a virus and about 15% are associated with Streptococcus infection (strep throat). In the United States, children typically average five sore throats per year and Streptococcus infection every 4 years. Adults typically experience two sore throats per year and Streptococcus infection approximately every 8 years.

    FOR SORE THROAT
    NATURAL TREATMENT AND COMFORT MEASURES
    • GARGLE WITH WARM SALT WATER. It help soothe the irritated throat and reduces swelling in the tissues. This is the safest, least expensive and probably the most effective treatment of a sore throat. Mix 1/2 tsp. of salt to an 8 oz. cup of warm water. Dissolve it completely. Gargle every 3 - 4 hours.
    • SIPPING WARM LIQUIDS like hot tea or hot chicken soup broth can soothe the throat, and the heat increases the circulation to the throat to promote healing. The saltiness of the soup also helps to reduce swelling, much like a salt water gargle.
    • HARD CANDY can also soothe and lubricate your throat.
    • REST YOUR VOICE. Avoid using your voice for long periods, and refrain from screaming or yelling.
    • STOP SMOKING or at least cut down. Cigarette smoking may cause a sore throat, or may aggravate your symptoms by causing further throat irritation.
    • HUMIDITY will keep your throat moist and more comfortable. It can relieve a raw, dry, scratchy sore throat. Hot, dry air will aggravate it. So, turn down the thermostat, use a vaporizer if you have one, place a pan of water by the heat vent, or turn on a hot shower to create steam.
    MEDICATIONS
    • CHERRY SORE THROAT LOZENGES contain Benzocaine which numbs the throat tissues temporarily and make swallowing easier.

Viruses are more common in children under a year. About 10% of sore throats are caused by the strep (streptococcus Group A) bacteria. This does need treatment with antibiotics because of rare complications of an untreated strep bacteria such as rheumatic fever (which can lead to heart damage) or post-streptococcal glomerulonephritis ( a kidney complication where blood and protein are detected in the urine and causes a "coca-cola" colored urine). Strep throat may occur at any age, but is most common in children over 4 or 5 years of age. It is contracted by person-to-person contact and the incubation is 2 to 5 days. Strep rarely causes nasal congestion and cough. Symptoms of strep throat may include the following: fever sore throat with red spots or white spots on the tonsils swollen tender neck glands headaches stomachaches, nausea or

Antibiotics for sore throat (Cochrane Review)
Del Mar CB, Glasziou PP, Spinks AB ABSTRACT A substantive amendment to this systematic review was last made on 28 June 2000. Cochrane reviews are regularly checked and updated if necessary. Background: Sore throat is a very common reason for people to attend for medical care. It is a disease that remits spontaneously, that is, 'cure' is not dependent on treatment. Nonetheless primary care doctors commonly prescribe antibiotics for sore throat and other upper respiratory tract infections. Objectives: To assess the benefits of antibiotics in the management of sore throat. Search strategy: Systematic search of the literature from 1945 to 1999, using electronic searches of MEDLINE (using the keywords, "pharyngitis", "sore throat" and "tonsillitis") after 1966, the Cochrane Library, the Cochrane collection of hand-searched trials, and the reference sections of the articles identified. Abstracts of identified articles were used to determine which studies were trials. Selection criteria: Trials of antibiotic against control with either measures of the typical symptoms (throat soreness, headache or fever), or complications (suppurative and non-suppurative) of sore throat.
