For people with hypernasal speech, many of them with repaired cleft palates, there have been few remedies other than surgery, and the surgery comes with risks. Some of these people, however, may find help in a kind of weight-lifting for the soft palate, says David Kuehn, a professor of speech and hearing science at the University of Illinois. Making use of a device for treating sleep apnea, Kuehn devised a treatment almost a decade ago to strengthen the muscles of the soft palate, which acts as the door between the oral and nasal cavities. To produce most language sounds, the door should be closed, but people with hypernasal speech are unable to close it completely. Stronger muscles, Kuehn reasoned, might make it possible for subjects to close that door more tightly, reducing their nasality and getting closer to normal-sounding speech. With results from a study completed last year, Kuehn now has evidence his therapy works. "For the first time, we have hard data to back up a statement that this therapy does, in fact, statistically reduce hypernasality in a fairly large group of subjects," he said.

Standard treatment for phobias calls for the patient to gradually face the fear, by either imagining it or confronting it. The appeal of VR therapy is that it's more realistic than visualization therapy and more practical than real-life exposure. (Just try getting 25 of your closest friends together to hear your quarterly sales presentation.) While there are still no large-scale studies of the efficacy of the approach, psychologists and other experts seem excited. "I was quite impressed when I saw it," says Dr. Stefan Hofmann, who heads the social-anxiety program at the Center for Anxiety and Related Disorders at Boston University. "But we really don't know how effective it is yet." Dr. Robert Reiner, a Manhattan-based psychologist who once feared public speaking himself, heads Behavioral Associates, an institute specializing in cognitive behavioral therapy. It is also easy to use, though the controls are a bit fiddly. It works well for dictation if you hold it about four inches away from your mouth: further away, you may not get a comfortable volume. It also works well at a few feet if you switch to conference mode, but the higher microphone sensitivity naturally picks up more background noise. It does the job, though it would probably do it better with an external microphone. However, sound quality is much lower than you get with a good cassette recorder or MiniDisc recorder, and is also mono. This comes from using DSS (Digital Speech Standard) files, which are much more compressed than MP3 or WMA (Windows Media Audio). The advantage is that you get about 2 hours of recording time in standard mode, or five hours in LP mode. But I would have liked the options to save in other formats, and an SD slot to expand the memory.
