News : Aneurysm screen lifesaver Screening men for abdominal aneurysms can cut deaths An MRC funded study has shown that ultrasound screening to diagnose abdominal aortic aneurysms (AAAs), which are potentially fatal balloon-like swellings in the wall of the main artery that runs from the heart through the abdomen, can cut death rates in those screened by around half.
The Multi-centre Aneurysm Screening Study (MASS) at hospitals in Southampton, Winchester, Portsmouth (including Hasler Naval Hospital) and Oxford involved 67,000 65-74 year old men, who were divided into two groups; those who were invited to be screened and those who were not.
Ultrasound tests detected 1333 AAAs in the screened group, which suffered 65 aneurysm related deaths compared to 113 AAA deathsin the non-screened control group. The results were published in the Lancet on Friday 15 November 2002.
If an aneurysm ruptures the chances of survival are low, with half the patients never reaching hospital. Currently the only effective treatment is to repair them by surgery and this is usually done when the aneurysm is over 5.5cm in diameter.

Aneurysms in the brain occur when there is a weakened area in the wall of a blood vessel. They may occur as a congenital defect or may develop later in life.
A saccular aneurysm (berry aneurysm) can vary in size from a few millimeters to over a centimeter. Giant berry aneurysms can reach well over 2 cm. The aneurysm resembles a sack of blood attached to one side of the blood vessel by a narrow neck. These are more common in adults. Multiple berry aneurysms are not unusual. About 25% of patients have more than one aneurysm. They can occur in any blood vessel which supplies the brain, but they are most often seen in the large arteries at the base of the brain. Berry aneurysms are associated with polycystic kidney disease and coarctation of the aorta . Rarely, berry aneurysm can run in families.

"Abdominal aortic aneurysm" isn't a condition you hear about everyday. Yet two Mayo Clinic studies document a three-fold increase over the past 40 years. The increase may be partly due to the upsurge in smoking since World War II. Also, as more people live longer, this type of aneurysm occurs more frequently. About 15,000 Americans die each year from ruptured abdominal aortic aneurysms. Compared with the half million who die annually of heart attacks, the number of deaths due to aneurysms is small. But it's scary that an abdominal aneurysm may rupture and cause sudden death. If detected early, however, surgery eliminates this silent danger 95 percent of the time.

A dissecting aneurysm indicates a hemorrhagic separation in the aortic wall, usually within the medial layer. A saccular aneurysm describes an outpouching of the arterial wall, with a narrow neck. A fusiform aneurysm is a spindle shaped enlargement encompassing the entire aortic circumference. Thoracic aortic aneurysms are most common in men between ages 50 and 70. What is an aneurysm?
An aneurysm is the dilation thinning and ballooning or bulging out in part of the wall of a vein, artery, or the heart. An aneurysm may be small and not cause symptoms. What causes an aneurysm to form?
An aneurysm may be caused or aggravated by disease (such as atherosclerosis or hypertension), injury, or an abnormality present at birth. How are aneurysms found?
They are found by imaging techniques such as echocardiography, or MRI or CT. When they are found, they should be regularly checked. What dangers do aneurysms present?
The danger from an aneurysm is that it will continue to bulge and may burst. When an aneurysm in a large blood vessel or in the heart bursts, a person could bleed to death. When an aneurysm bursts in the brain, a stroke (brain attack) can result.

The wall of the aneurysm stretches and is thinner and weaker than the rest of the artery wall. What happens when an aneurysm ruptures?

An aneurysm is a dilatation of a blood vessel, almost always an artery, to the point where it becomes stretched out and begins to look like a balloon. This usually occurs because of hardening of the arteries (atherosclerosis) or general deterioration of the arteries that can come with aging. Aneurysms often run in families as well. Aneurysms are dangerous because when the artery stretches, it becomes weakened. A major source of complications or even death from aneurysms is when they rupture, or the wall of the artery breaks. This is more likely to happen in larger aneurysms. Rupture of an aneurysm can cause major and fatal hemorrhage (bleeding). Vascular surgeons are experts in treating aneurysms in many parts of the body. The most common locations of aneurysms which need to be treated are the aorta (the major blood vessel located in the abdomen) and the legs. Aneurysms can be diagnosed when an abnormally large pulsation or mass is found when a patient is examined. Vascular surgeons are expert in detecting aneurysms by the physical examination of patients alone. Aneurysms can also be seen on many types of X-rays or imaging studies (for example, CT scans, sonograms, MRI). The goal of finding aneurysms is to treat them before any complications, particular rupture, occur. Once an aneurysm is found, a decision is made as to whether it needs to be treated, or whether it can simply be watched. Almost all aneurysms can be treated routinely with standard vascular surgical techniques. More recently, we have become specialists in the newer field of "endovascular surgery", which allows us to treat many aneurysms using less invasive methods. This allows a shorter hospital stay and easier recovery for the patient.

Cerebral aneurysms are areas where a blood vessel in the brain weakens and "balloons." They are the result of defects in the elastic layer of the blood vessel wall. Aneurysms usually develop where arteries branch. The fork formed where arteries branch may be weaker than other areas. When pulsating blood pushes this area of weakness outward, an aneurysm results. Eventually, the aneurysm may balloon and thin the wall to the point that it ruptures. What are the signs and symptoms of the condition? Many aneurysms cause no symptoms until they get quite large or rupture. Bleeding from a ruptured aneurysm generally produces a sudden, severe headache . Headache is usually followed by neck pain and neck stiffness that occur when blood causes an irritation of the meninges. The meninges are the protective layers covering the brain and spinal cord. If the amount of blood in the space under the meninges is large, other blood vessels may go into spasm. A large blood clot can also press on surrounding brain tissue. When this happens, the person may show progressive drowsiness . Other symptoms, such as paralysis and visual impairment, can also develop.
